• The first step is to call Patient Partner to schedule an in-home appointment at your convenience. Please call us at 215-335-9290. Ask for Fran or email Fran at fran@patientpartner.com.
• The second step is to perform an assessment. The assessment will be based upon your needs and wishes. At this meeting you will meet with a Personal Care Coordinator.
• The third step is to design a Care Plan that addresses your needs and wishes. The Care Plan will outline the scheduling and type of  services.
• The fourth step will review the cost of the services and the method of payment.
• The fifth step. We will introduce you to your Patient Partner team.
• The sixth step. Congratulations! You now have a “trusted partner” to deliver the promise of worry-free independence at home.
