Our (HHP) is designed to manage and administer your changing needs as you age. Our philosophy is that there are numerous issues that must be addressed in order to maintain your independence at home. We believe it is important that these services be rendered by a trusted partner. This important role as a trusted partner is why we chose the name Patient Partner.

If you and your spouse have no plans of living in a nursing home at this time; partner with us and enjoy the benefits of worry-free independence. Our comprehensive approach will address your medical, non-medical and home maintenance concerns. We strongly believe our safety net concept is far superior to any other approach.

How much does the HHP cost?

The monthly membership fee is $250.00 per individual and $350.00 per couple. As you can see, our approach is affordable and the value is second to none. In the event, you are not interested in the membership approach; please note we do provide services on a fee-for-service basis.
